I obtained an extended order of OP for now. The judge did grant that the other parent undergo a drug/alcohol assessment with testing and also supervised visits that have been arranged. The other parent has 30 days to comply with the assessment at which time I'm sure he will petition for a visitation change. My concern is that he has always denied any problem and will only straighten up for a short period of time with his 35+ year history of addiction. I want to petition for a longer period of drug/alcohol testing and/or counseling with continued supervised visits for, at least, a period of 6-12 months until, hopefully, I'm okay with him around the children. Any ideas of what I must present to the court? I already have documented exposures around the children but his last DUI/drug arrest was almost 3 years ago. Any other suggestions?

Just know that the judge will in all likelihood bend over backward not to interfere with Dad's visitation time. So the odds are against you.
I would start with common sense. It's clear that you still are uncomfortable trusting Dad around the children. Why is that? Once you know the answer, you know the evidence you need to share with your lawyer. If your reason is nothing more than "I know he's no good," you'll have a tough time winning the judge over to your point of view. But if you can document a series of times when Dad has won people's trust and then betrayed that trust, particularly if those behaviors involve danger to the children, you may be able to help the judge understand why it's just a matter of time before Dad trips up again.
